Camper Forms & Applications

Applying for Camp is easy. Just follow these instructions. To learn more about a particular program, click on the Camp's title. And if you have any questions, please feel free to contact us.

Instructions

  1. Download all forms required for each camp. For example, if you plan to attend the Young Adult Conference, you must download the participant application, the physical exam form. If you cannot view or print these documents, you might not have a PDF reader installed on your computer. Visit the Adobe Web site for a quick, free download of Acrobat Reader.
  2. Print them out and write/type in the information.
  3. Sign the application and all forms, and have your parent(s) or guardian sign them, if applicable. Have your physician complete the Physical Exam Form.
  4. Mail or fax forms to Camp Mak-A-Dream at
    P.O. Box 1450, Missoula, Montana 59806.
