DATES & APPLICATIONS
To apply for any of our camp sessions, please follow the
instructions below.
Please note that while applications may be sent it at
any time throughout the year, the selection process for our summer
sessions will not begin until April.

The forms are available for download in Adobe Acrobat PDF format (go
to the site viewing requirements for
more information on downloading these documents).

Download all of the forms listed for
each camp. For example, if you plan to attend the Young Adult
Conference, you MUST download the participant application, the
current treatment form, and the physical exam form.

Print out each form, and provide all of the
information asked for on the forms.
or
Fill in the forms online by using the <Tab> key to go from
field-to-field, and print out the completed forms.

Sign them and have your parents or
guardian sign them, if applicable.

Return all of the forms to Camp Mak-A-Dream at the
address listed on the bottom of the form.
