Applying for a spot at Camp is easy. Just follow these instructions. If you have any questions, please feel free to contact us.

Instructions

1. If you cannot view or print this document, you might not have a PDF reader installed on your computer. Visit the Adobe Web site for a quick, free download of Acrobat Reader.
2. There are two ways to fill out the application. You can print it out and write in the information by hand. Or, fill in the information on your computer by using the TAB key to go from field to field, then print out the completed application.
3. Sign the application.
4. Mail or fax the application to Camp Mak-A-Dream using the contact information listed on the bottom of the form. Other application materials (letters of reference, physical exam form, etc.) will be requested after the interview, should you be accepted for a position.
